Participation with a club carries with it a great deal of personal responsibility. As a representative of The University of Arizona, club members are expected to conduct themselves in the most positive way possible at all times. All members of Ritmos Latinos are required to adhere to all federal, state, and University laws, guidelines, standards and policies. Ritmos Latinos reserves the right to issue sanctions, including but not limited to denying or revoking membership to individuals in violation of University policies. Please initial the box next to each term below to agree to that individual term. (You will not be allowed to continue until all terms are agreed upon.) CLUB MEMBERS, OFFICERS, AND INSTRUCTORS SHALL NOT: •Engage in any form of hazing. Hazing is any action taken or situation created (the willingness of an individual(s) to participate notwithstanding) upon which initiation, admission into, or an affiliation with an organization is directly or indirectly conditioned and which produces mental or physical discomfort, embarrassment, harassment, or ridicule. •Use drugs (except for medical purposes) while practicing, traveling or competing. •Consume alcohol while traveling, practicing, competing, spectating or socializing as part of a club event. •Consume alcohol at any University of Arizona facility before, during, or after a class session. •Use club funds to purchase alcohol. •Encourage underage consumption of alcohol. •Attempt to strike, threaten or cause personal injury to anyone while practicing, competing or spectating. •Verbally abuse another participant, instructor, spectator, administrator or hotel employee. •Cause damage to facilities or equipment. Use or enter a facility illegally, using an assumed name of fake I.D. •Use obscene gestures, profanity or disrespectful language while practicing, competing or spectating. •Engage in conduct that constitutes discrimination or harassment. The above is a partial list of expected standards of behavior for participants. For additional information concerning standards of behavior, refer to http://policy.arizona.edu/ethics-and-conduct. Ask for the Anti-Harassment Policy enforced at the club if you have more questions. As a member of the club Ritmos Latinos, I agree to the guidelines outlines above. I have read and fully understand this agreement and my submission of this form confirms my understanding that I can be subject to probation, suspension or termination from the organization/club for any violation of the agreement. Failure to agree to this Code of Conduct disqualifies a student or an instructor from any involvement/ participation in further activities of the club Ritmos Latinos. I hereby agree as follows: 1. Risks of Participation. I fully recognize that there are dangers and risks to which I may be exposed by participating in the Program. The following is a description and examples of specific, significant, non-obvious dangers and risks that may be associated with the Program: consenting to being driven by a UA/Ritmos Latinos volunteer in their personal vehicle. 2. Health & Safety. I understand and agree that the University of Arizona (the “University”) and its governing board, officers, employees, and agents including Ritmos Latinos executive committee, instructors, volunteers and/or members (the “Releases”) do not have medical personnel available at the Facility, which is the site location for my participation in the Program. I understand and agree that the Releasees are granted permission to authorize emergency medical treatment, if necessary, and that such action by the Releasees shall be subject to the terms of this Agreement. I understand and agree that the Releases assume no responsibility for any injury, damage or cost which might arise out of or in connection with such authorized emergency medical treatment. I have consulted with a medical doctor with regard to my personal medical needs. There are no health-related reasons or problems which preclude or restrict my participation in this Program. I have arranged, through medical insurance or otherwise, to meet any and all needs for payment of medical costs while I participate in the Program. I understand that neither the Releasees nor the Facility are obligated to provide transportation in connection with the Program. I understand that I am expected to carry my own automobile liability insurance coverage. 3. Standards of Conduct. I will comply with the University's rules, standards and instructions for student behavior, as well as the Facility’s standards of conduct. I waive and release all claims against the University that arise at a time when I am not under the direct supervision of the University or that are caused by my failure to remain under such supervision or to comply with such rules, standards, and instructions. I agree that the University has the right to enforce the standards of conduct described above, in its sole judgment, and that it will impose sanctions, up to and including expulsion from the Program or from the University for violating these standards or for any behavior detrimental to or incompatible with the interest, harmony, and welfare of the University, the Program, the Facility or other participants. The University/Ritmos Latinos has the right to make changes in the format and administration of the Program. I understand that the University has no control over the operations or premises of the Facility, and that I will be under the supervision of a Facility representative while I am participating at the Facility. 4. Assumption of Risk and Release of Claims. Knowing the risks described above, and in consideration of being permitted to participate in the Program, I agree, on behalf of my family, heirs, and personal representative(s), to assume all the risks and responsibilities surrounding my participation in the Program. To the maximum extend permitted by law, I release and indemnify the Releases from and against any present or future claim, loss or liability for injury to person or property which I may suffer, or for which I may be liable to any other person, during my participation in the Program (including periods in transit). 5. Multi-Media Consent and Release. I grant the Arizona Board of Regents and Ritmos Latinos on behalf of the University of Arizona the right to videotape, film, audio record and/or photograph me during my participation at the club. I agree to release the University/Ritmos Latinos from any and all claims for compensation, libel, false light, invasion of privacy, moral rights and rights of publicity. 6. Anti-Harrasement Policy: I commit to create a respectful, healthy, and safe environment and abide by the UA’s Non-Discrimination and Anti-Harrasment policy guidelines (as mentioned at https://titleix.arizona.edu/ua-policies/nondiscrimination-anti-harassment-policy). If I am found not following the guidelines, any actions taken by the Executive Committee and/or the University of Arizona will be final and not for dispute. I have carefully read this Release Form before signing it. No representations, statements, or inducements, oral or written, apart from the foregoing written statement, have been made. This agreement shall be governed by the laws of the state of Arizona, which shall be the forum for any lawsuits filed under or incident to this agreement or the Program.
